The Community Partnerships team is committed to improving health and wellbeing outcomes for the region through genuine partnership, shared decision making and working collectively to achieve agreed outcomes in areas of shared priority. Our team also works to ensure public health stakeholders and partners are supported to achieve the best possible health outcomes for communities.
Our goal is to build and support a public health ecosystem that is networked, impactful and responsive and has a health equity lens applied to all programs and activities.

The BSWPHU Community Partnerships team has a prevention and population health focus and works collaboratively with key prevention partners across the Barwon South West region.
The Barwon South West region comprises a population of nearly 460,000 people across 10 local government areas, spanning from Queenscliff Heads to the South Australian border. Traditional land owners are the Bunganditj, Djab wurrung, Djargurd wurrung, Gadubanud, Kirrae wurrung, Gulidjan, Gunditjmara, Jardwadjali, and Wadawurrung Aboriginal people.
We work closely with health services, local government, women’s health services, and other not for profit agencies including Aboriginal Community Controlled Organisations.
This map provides a visual overview of the Registered Aboriginal Parties across the Barwon South West region, with local government areas and towns included for context. It was developed to build knowledge and confidence in understanding the Traditional Owner groups connected to the areas where we work.
BSWPHU Strategic Plan
The Public Health Strategy for the Barwon South West Region 2023-2033 articulates a pathway to realise the regions vision that communities across the Barwon South West achieve their best possible health and wellbeing.

Catchment plan
The BSWPHU Population Health Catchment Plan 2023-2029 identified priority areas for action over six years. The Catchment Plan supports the goals and objectives of the overarching Barwon South West Public Health Unit Strategy.

Annual Implementation Plan
The Annual 2025-2026 Implementation Plan is designed to sit alongside the Barwon South West Public Health Unit (BSWPHU) Population Health Catchment Plan 2023-2029. The priority areas of the Implementation Plan were developed during the consultation phase of the Catchment Plan. We have continued to refine the strategies and short term outcome measures in consultation with our partners.

The key roles of the BSWPHU in delivery of the strategies in the Implementation Plan are to:
| Lead | BSWPHU will facilitate a region wide approach to prevention and population health which is informed by local priorities and capacity. |
| Support | BSWPHU will work alongside partners and stakeholders to add value to their local work bringing BSWPHU skills, knowledge and resources. |
| Connect | BSWPHU will enhance the local prevention system by connecting partners to each other within the region and to external support systems and mechanisms. |
| Promote | BSWPHU will work with government, partners and stakeholders to build awareness of programs, projects and opportunities. |
